The plant foot:

Your foot is overturned when it plants. Instead of planting at a 90 degree angle (or more)  to your line you are probably only at 45 or so.

I think it is a symptom of opening your hips too early and it is resulting in a significant loss of power.

 Only thing I really notice is maybe a little drop in your elbow...

You need to shave, lose a little weight, and most importantly you are throwing with the wrong hand.

Trying to compare your throw to the David Feldberg video has helped...  I have noticed a few things I do wrong by watching those videos then watching mine.  I see I have been breaking my wrist too early.

I went out during lunch to see if I could turn what I learned into distance and consistency and I was able to achieve a noticeable distance gain on my throw but more importantly my accuracy was greatly improved!  Community breakdown of a slow motion video is a great tool!!

And I think one of the best things about video study is that you get to see yourself throwing. It let's you see all sorts of things. Outside help is great too, but don't underestimate what you see.

Do you essentially mean that I'm taking the elbow off line? I'm just having trouble visualizing the problem/figuring out how to fix it.

One of the things I noticed in it is that I don't have a full reach back and/or I have insufficient weight shift back. Anybody else agree with that thought?

Yes, I agree, Cooper.  The first thing I noticed is that you don't have a full reach back.

I've gone with Will's method -tought to me personally by Will *cough, cough* - and I find it provides increased straight line accuracy and power.

I stopped the full reach back from a Hemme suggestion.  I lost some distance, but I gained more command.  I am not as accurate or consistent with a full reach back.  Heck, I am not accurate or consistent.  I would love to incorporate more reach back into my game.  I would love to incorporate more consistency  and accuracy.  I would love...  Suggestions on how the heck Will throws with full reach back and accuracy is appreciated.

I think first we need to address how far you want to throw.  Regardless of what most people lead you to believe, less than 1% of all disc golfers throw over 400' consistently on flat ground.

Personally, I think getting accuracy inside 350' is the most important thing before trying with a full reach back.  Generally, you need a bigger reach back to throw midranges north of 350' but you should be able to control a driver with shoulder turn and minimal reach back in that distance.
